Overview
This contract is to ensure support for an individual requiring placement in an independent living setting is secured and works cohesively with the on-site care provider. Location is an important factor in this contract, fulfilling the individual's wishes. The placement will cover their living arrangements and ensure that the individual has suitable accommodation to live in the least restrictive way. The service will include: Accommodation provision with emergency on-site support Interlinking with the on-site care agency who will be providing support Ongoing maintenance of the accommodation provision This contract is a continuous contract with ongoing rolling terms. The contract will terminate when all individual placements made through this contract are ended (known as the Individual Service Agreements).
